As the director of the second largest VA healthcare system in the nation, Cave is responsible for leading facilities in Dallas, Bonham, Fort Worth, Tyler, Plano, and five community-based outpatient clinics throughout its 40-county service area. With an annual operating budget of approximately $1.8 billion, 6,600 staff, and 1,500 volunteers, VA North Texas Health Care System provides care to almost 206,000 enrolled veterans. 

Previously the chief executive officer of the West Texas VA Health Care System in Big Spring, Cave, a general attorney, had been the acting CEO for South Texas Veterans Health Care System in San Antonio since June 2022. Cave has held key leadership positions within the Department of Veterans Affairs since he joined VA in 2011 as a Presidential Management Fellow. He has served VA Medical Centers of all complexity levels across three different Veteran Integrated Service Networks (VISNs) as well as VA Central Office and has performed in a variety of capacities to include health systems specialist, financial manager, general attorney, public affairs officer, and emergency manager. 

Cave is a graduate of VISN 4’s Leadership Development Institute Class of 2013, VA’s Leadership VA Class of 2016, and Veteran Health Administration’s Healthcare Leadership Development Program class of 2019. He holds an undergraduate degree in accounting from the University of New Mexico and a juris doctor from the University of Akron School of Law in Akron, Ohio, including certificates in trial advocacy and dispute resolution.
